2,500 yen (tax included)

□Notification of election or non-election

□Regarding the Decision to Hold the Event
In case of bad weather or other circumstances, the event may be canceled.
In such cases, a decision will be made one hour before the event begins. Only if the event is canceled, an announcement will be made on the FC Tokyo Park Fuchu website and Twitter. Please note that we will not contact you directly regarding cancellations.

□Notes on Using the Facility
・Please do not bring your own balls (the facility will provide them).
・The facility is located on the rooftop of a building. Balls may not be used anywhere other than on the pitch.
・Spiked shoes are not permitted on the pitch. Please wear training shoes or futsal shoes.
・There may be times when it is difficult to reach us by phone. Thank you for your understanding.
・Photographs may be taken during the event.
They may be used by FC Tokyo or FC Tokyo Park Fuchu. Thank you for your understanding.

□Others
・If you are a member of a team, please be sure to obtain the approval of your team before applying.
※We will consider that you have obtained the team's consent at the time of application.
・If we take photos during the event, they may be published in various FC Tokyo promotional media. Please understand in advance.
・Regarding personal information such as your address and name, we may use it to contact you and provide information about FC Tokyo Promotion Department.
